What certifications does HOPE have?
HOPE remains accredited to provide services through the Medicaid HCSB Waiver, and we possess a three-year accreditation from CARF. We are also contracted with Polk County Health Services as a PCHS Network Provider.

Who qualifies for our services?
​Individuals must qualify for the Intellectual Disability (ID) , Brain Injury (BI) Medicaid Waivers or have a diagnosis of a developmental disability that qualifies for funding through the County.  Services then are based on needs and aspirations.

Where can services be provided?
​All services are provided in the community, place of employment or in the individuals home.
