HOPE is proud to offer the following services to our participants and their families:
  • Supported Community Living (SCL) - promotes skill building services that are unique and individualized to each participant. The goal of SCL is to equip those receiving services with tools and techniques to help gain and maintain greater independence in their homes and communities. A few examples of participant driven goals include cooking, social skills, budgeting, cleaning, and community involvement. 
  • Supported Employment (SE) - promotes skill building services in paid work environments. HOPE is able to assist and support individuals at various points on their employment journey. A few aspects of SE include the following supports: resume skills, job interviewing skills, acquiring paid employment, and support in maintaining employment.
  • Customized Employment - flexible process designed to create a personalized employment relationship between a job seeker and a business in a way that meets the need of both. Customized Employment is an individualized approach to employment planning and job development - one person at a time and one employer at a time.
  • Discovery - the foundation of job development for those needing extra support in seeking employment. Discovery stats with the job seeker, not the potential employer. It is a functional assessment designed to learn who the job seeker is, taking an inventory of that person's skills, interests, experiences, relationships, abilities, and creating a positive personal employment profile. This profile underlies the plan to contact businesses where the job seeker and company both benefit when the individual becomes an employee.
  • Respite - a service that allows the primary care taker to take a break. The goals are not related to skill building, but rather allows individuals to spend one-on-one time with a direct care professional in the community or the individual's home. 
  • Supported Education-  provided to individuals enrolled in post-secondary institutions or other employer recognized certificate program.  Including HiSET (formally GED), career exploration and planning, assistance in obtaining tuition and related expenses, application process, study strategies, assistance in obtaining tutoring and accommodations, and other supports needed that interfere with educational success.

A unique component to HOPE's delivery of service is the holistic wrap-around philosophy. All services HOPE provides start with the SCL component. As we get to know the individual, we might find that we are able to offer another aspect of services that will further support in reaching their goals and aspirations.

Frequently Asked Questions:

What certifications does HOPE have?
HOPE remains accredited to provide services through the Medicaid HCSB Waiver, and we possess a three-year accreditation from CARF through 2022. We are also contracted with Polk County Health Services as a PCHS Network Provider.

Who qualifies for our services?
​Individuals must qualify for the Intellectual Disability (ID) , Brain Injury (BI) Medicaid Waivers or have a diagnosis of a developmental disability that qualifies for funding through the County.  Services than are based on needs and aspirations.

Where can services be provided?
​All services are provided in the community, place of employment or in the individuals home.
