A unique component to HOPE's delivery of service is the holistic wrap-around philosophy. All services HOPE provides start with the SCL component. As we get to know the individual, we might find that we are able to offer another aspect of services that will further support in reaching their goals and aspirations.

What certifications does HOPE have?
HOPE remains accredited to provide services through the Medicaid HCSB Waiver, and we possess a three-year accreditation from CARF through 2022. We are also contracted with Polk County Health Services as a PCHS Network Provider.
Who qualifies for our services?
Individuals must qualify for the Intellectual Disability (ID) , Brain Injury (BI) Medicaid Waivers or have a diagnosis of a developmental disability that qualifies for funding through the County. Services than are based on needs and aspirations.
Where can services be provided?
All services are provided in the community, place of employment or in the individuals home.
